Promting for a username and password?

I am generating a series of pl/sql Procedures (in a package) that allow a user to view and update an oracle table interactively through the web. Does anyone have any suggestions on how to go about setting up a procedure that will ask the user to identify themselves by entering a username and password?
Also, has anyone had any luck exporting Oracle table info to a template that is readable in Word?
If anyone has any examples of code for this it would be greatly appreciated.
