Providing a good set of comprehensive questions is difficult - many sites have different requirements for DBA's. A suggestion on this note would be to pick up a copy of 'Sybase SQL Server survival guide' and take a look through some of the sample CSP questions (CSP - Certified Sybase Professional).

Alternatively, something I use is to print out an errorlog & config file from a Server, and ask the candidate to talk through any problems or items they might see. It could be a completely clean boot, but it forms a good point to start talking around their knowledge (Raw vs. cooked fs, I/O pools, Memory sizes, etc. etc.).
