HELP - Can't save library from Forms 4.5

I am having a real tough time saving a library from Forms 4.5 for Windows. When I go to save the library I get an error alert that says: 'PDE-PLI018 Could not find the library <filename>'. If I try to save the library again, I see that the library actually was saved (at least partially) because it shows up in the file save dialog. If I choose to replace the file I end up getting a GPF. Can someone help me please. I really need to create a library!
