This is what I do to create a second instance name "TESTINSTANCE"...
set your environment to the new SID i.e., setenv ORACLE_SID TESTINSTANCE

then sqldba and startup.

This method will try to read the $ORACLE_HOME/dbs/initTESTINSTANCE.ora file for the intialization parameters.
