Forms 4.5 GPF.. FAQ... help!!!

I am experiencing some frustration in Forms 4.5. As I am working in a form, I am frequently encountering a GPF in f45des.exe with tkwin20.dll as the culprit. Does anyone have a FAQ, or have a solve that I can use? I am at a loss. It is effecting my productivity!!
It occurs when running forms locally or off the network.
I am running netware 3.12, and win 3.1
