I have worked on several projects where converting from Forms 3.0 to 4.5 was the main focus. I can tell you that you are going to have to pretty much start from scratch and rebuild your whole application. The conversion utility that Oracle provides does not take advantage of the visual ability of the new tool or the new functionality 4.5 provides.
In both instances we found that it was actually faster to regenerate our modules from the CASE tool and rebuild them than to run the conversion utility.
All the projects involved over 200 modules that all had to be rebuilt and it took a while to get it done.
I hate to be the bearer of bad news, but these are real life stories.
