Currently Oracle is shipping 4.0 of forms for the Mac (68K only). The PowerMac version (4.5 Forms,et. al.), I'm told, has gone to manufactuering and should be shipping any day now. The 68K version of Forms 4.5, et. al., is due to be shipped maybe in April (don't hold your breath).

All of these products include the Runtime versions.
