This problem is usually caused by failing to SAVE the diagram before CLOSE or EXIT. (This is a known bug).

Be sure to FILE -> SAVE *before* FILE -> CLOSE or FILE -> EXIT.
Do regular FILE -> SAVE AS operations to back up your diagrams.
