Help ! SQLMenu runtime error.

I encounter a runtime error for SQLMenu. Whenever the menu item activates a SQLForms, I receive error like
No active item for root menu.
Ever I receive error like this, I can still activate the SQLForms successfully.
Any one can explain this.
