In forms 4.5 go to TOOLS-->OPTIONS look at module access. You can choose to save your modules to the file system or the DB. Note, their are some sql scripts that need to be ran by your DBA before you can save forms to the database. These scripts are:

                     FRM45BILD.SQL  (Creates Forms 4.5 DB Tables)
                     FRM45GRNT.SQL  (Used to grant access)
