Forms 4.5 :Creating items dynamically ?

Hi

I am having problems creatting an item at run time. i.e. I want to create a text item at run time at the click of a push button. I have tried it out in Other tools and I could do it. I tried the same way , but of no use. Can any one help me out wit this.
