Two things you want to look at. 1. Reconfigure your kernel to allow more semaphores (SHHMAX,SHMSEG or whatever the Oracle manual says, double them probably). 2. Check your /tmp device to see if any files associated with shared segments are left hanging around. If so, remove the old ones as part of your Oracle startup. Don't mess up anyones vi temp files!
