Best practices - test servers

What are you doing in your organization about test servers and procedures associated with development, testing, adn quality assurance? Currently, we are using a single test server that mimics our other servers (many, many tablespaces here), but we are still running into issues about control and change management. Let me know of any guidelines you use, or any resources for researching this further.
