There is a bug in Forms 4.0.something (probably what you have) that causes that error. Do the following.

1. Create the directory /usr/lib/X11/nls (as root) --this is the bug fix
2. Set the environment var DISPLAY=hostname[or ip addr]:0.0
3. Make sure that your LD_LIBRARY_PATH var includes /usr/lib and /x/oracle/vx.x/lib along with /usr/dt/lib This is assuming you are trying to run Forms on the machine which it is installed. If you are trying to run it from a workstaion and Forms is installed on a server, you need an extra motif piece for the workstation (about $100).
