Bug in Forms Designer for SunOS 4.1.3

Hi,
We have installed CDE 1 (Forms 4.0) for SunOS 4.1.3. When starting the froms designer (f40desm executable) it comes up with damaged tools palette at the left edge of the window.
Actually, all the icons (of pushbuttons etc.) are not seen, but when I start clicking them by mouse, they are discovered. Unfortunately, this is only for a while: after some operations, the designer window appears without any seen icon again.
Is it a know bug?
Any idea how to fix it?
