Forms 4.5 Disconnect Error

Last week I installed Oracle Forms version 4.5.5.2.11 on my PC. The designer part of the program seems to work just fine, but I'm some problems with the Runform piece.

Specifically, when a runform sesssion(s) disconnects from the database it is generating errors when attempting to write the disconnect information to the audit table (we're auditing all connects and disconnects to the database).

The runform component (for some unknown reason) established two database sessions when a form is executed. The connections are via TCP/IP using SQL*net Version 2.0 with the multi-threaded server (I believe we're using Multinet TCP/IP on the server and Pathworks on the PC).

When the form is exited, the databsae attempted to generate an audit record of the two disconnects. This results in the following error:

ksedmp: internal or fatal error
ORA-00600: internal error code, arguments: [1237], [], [], [], [], [], [], []

I also eventually get the following error in my alert log:

found dead multi-threaded server 'S000', pid = (8, 2)

Anybody seen similar errors? Or can anaybody explain why runform needs two database session to run a form.
