Forms 4.0 problem, Help !

I am having problems with Forms 4.0, I am using:

ORACLE  7.0.16.4.1
FORMS   4.0.12.4.1
SunOS   4.1.3_U1

It crashes with memory fault a lot.

Any help will be appreciated.
