In reference to the problem in which performance in Forms was allegedly orders of magnitude off what SQLDBA was showing: It turns out the problem was the author's faulty emulation of the Forms generated sql in SQLDBA. Case insensitive query was turned on for the item qualifying the where clause [it puts UPPER() in the sql line].
