I think you better contact your DBA...Generally speaking if sqlforms30 is fully installed the following tables are accessible:

                      IAPAPP
                      IAPBLK
                      IAPCOMMENT
                      IAPFLD
                      IAPMAP
                      IAPSQLTXT
                      IAPTRG
                      IAPTRIGGER

If you are getting table errors while trying to create a new form these tables are either not created or you do not have grants on them...
