This happened to me during a period when the version of the database tables (4.0.12) was not "in sync" with the version of Forms I was running (4.0.11). Try re-running the "Tools Utilities" scripts to re-build the Forms tables (Save all forms to the file system first as this will wipe out all forms in the database).
