There is no GUI version of Forms 3.0 for the Oracle7 release. It was only put out on Oracle v6 on some platforms. On Oracle7, you should move up to RDBMS 7.0.16 and Forms 4.0. Note, however that Forms 4.0 is Motif-based. You will probably be able to run it using olwm as the window-manager; however, the application will still look Motif-y.
