ORA-08102 error in forms

Has anyone encountered oracle error

        ORA-08102: Index Key Not Found

We are ocassionally encountering this when inserting into a particular table. Most inserts are successful.

We've tried validating indexes, but found nothing wrong. The error message book says "send a trace to oracle". We are going through this, but are hoping for a more rapid response from this group.

Any insights?
