EIS Development Survey

I'm interested in hearing about experiences in development of Executive Information Systems. In particular:

-How did the project originate?
-What software/hardware was used?
-What role did the users play in the development process?
-Would you consider the finished system a success? Why/Why not?
-If, like me, you've been in this field longer than you care to think about,
  how does EIS development differ now from what it was 10-15 years ago?

This is more for personal interest than anything else, but if there's any interest I'll post a summary of any responses I get.
