Just a little SQL*Forms tidbit we discovered, today. Under VMS, we have SQL*Forms Design: Version 3.0.16.4.0 - Production

Copyright (c) Oracle Corporation 1979, 1992. All rights reserved.

Using Oracle Toolkit Version 01.00.19.00.00 (Production) Using PL/SQL Version 01.00.32.03.00 (Production) Using SQL*Menu Version 05.00.11.03.00 (Production)

Someone called me wanting to know why their PRE-COMMIT and POST-COMMIT triggers didn't fire during the commit process. It turns out that their triggers had been assigned to a block. (The only block in the form). After removing the block name from the trigger def screen the triggers began firing.
