we have had the same problem when running SQLPME 2.2.11 and Forms 3 on MSDOS 5. it seems that the Oracle recommendations for the extended memory requirements are a bit low.

Oracle recommend 2800k for SQL*Forms (Design), but i have found that forms needs about 300k more than that to run happily. i have never had a protection exception when running with 3100k free to SQLPME.

you may need to make sure that programs like smartdrive, ramdrive and emm386 are not eating all your free memory and not leaving you with enough for forms.

typically, is it recommended that you should allocate no more than 2Mb of ram to smartdrive on an 8Mb machine.
