table change distribution

We currently need to distribute table changes etc to 350 remote sites on a regular and controlled basis. Anyone like to share with me how they currently tackle such a job?

Some of the problems are that changes must be applied in order, new programs must be loaded in concert with table changes, table changes must be automatically applied to the database (updating 350 sites manually is not on).

Any feedback appreciated.
