oracle 9.0.4.1.0 beginner problem ora-00984

The problem is when I use the form insert and save it produces that error of ora-00984. All the fields are varchar2 that are displayed, but there are three fields not displayed 2 are dates and another is a varchar2. I hope that there is enough information to help me.
