What you have said makes little sense.

1. The application server should be started once at boot time when the AS box is brought online.
2. 10g Forms are webforms only. A webform does not talk directly to the DB
3. Forms & the DB require different ORACLE_HOME trees & have separate tnsnames.ora files. The fact that SQL*Plus works is independent to Forms.
