Which version of 9i are you using, release 2? If so, there is an port specific bug fix that needs to be applied. Check out the Release Notes for Dev6i Patch 12, I was just looking at these yesterday. The patch I'm refering to is 2530365 for Windows NT. "Forms Developer 6i may crash when connecting to an Oracle 9.2 database." This is directly from the white paper. Also, there are some requirements for JInitiator for with Forms 6i and iAS 1022 in that JDK 1.1.8 is desupported, you need to use JDK 1.2.
