Issue in Oracle Forms 6i

At various times as soon as the record is double clicked on B1, even before viewing the details on B2 the applications exits. I tried to recreate the block but still the problem exists. I cleaned up the data and created fresh rows but it did not help. The problem has no regular pattern, does not always happen, does not happen on the same record and is very erratic.
