Forms/Reports 6 problem - error.BMP (0/6)

I have an app that uses Forms v6 to retrieve data and then the user presses a button to print the data ( the button uses RUN_PRODUCT to call a report and pass it query parameters) ....It has stopped working, and I get the attached error message ( it's a screen print) - basically there is a failure to find an entry point in a routine and I'm at a loss to figure out what or where this routine is..
