I ended up calling Oracle Support on this one. They asked for my log files:
wrb.log
wrb.app
site.app
as well as output from running unixenv.
So far, I haven't received a response from Oracle on this. I can understand how such a broad/general problem description can sometimes be hard to diagnose and troubleshoot, so I'm remaining patient. In the meantime, it doesn't seem to be happening again.
