Terminal with HP-UX and Forms 4.5.7

I have a bug with Terminal 2.3, Forms 4.5.7 (for Unix HP) and vt220 terminal type, the workarround says I have to use the file fmrctv220.res or fmrctv100.res supplied with forms 4.0.13.17.2.

Can anyone send me this file, please?
