Let me know the versions:

1:NT 
2: OAS
3: Developer server

2: Try moving your window ( resize it - small to big)

3: make sure if you are using Netscape ( update the parameters between <embedded><\embedded>

4: Try static first, then try cartridge.

Worked for me finally ( nt 4.0/sp5, oas4.0.7.1, developer server 6.0.5.0.32 ( approx - latest patch).
