Need help on generating a menu form from DES2K2.1

Hi there,
Does anyone know how to generate a menu form from a module in Designer 2000 R2.1?
I was able to succesfully generate the main menu module and its submenus, I don't know how to generate the form which sole purpose will be to serve as a "Console" for the main menu. Designer doesn't generate it because it doesn't use any tables.
Any help will be gladly appreciated.
