I have had a canvas appear, then disappear right away, but this turned out to be a navigation problem... my SHOW_CANVAS displayed correctly, but it was immediately followed by the reset of the initialization of the module, which defaulted the first navigation to a field covered by the canvas, and so Forms hid the stacked canvas.
