I got this a few times in Reports 3.0 - I think there is a bug in this area and sometimes this trigger simply does not fire. I have seen this on the simplest of forms where I am inserting a record then committing. Sometimes the record does not get written to the database and sometimes it does. I have not been able to trace any kind of pattern in this effect though.
