Silly Form Builder question

I have created an LOV and I've dicking with this issue for like two days and driving me insane, I've been trying to make it work. It does not.

I have created an LOV and I've dicking with this issue for like two days and driving me insane, I've been trying to make it work. It does not.
It does not show the blessed LOV on the form. It shows something like a tabular column.
Can you please help me. Thanks a lot
