Looking for an example or template of an order entry form (Forms 5.x)

I am working on an order entry form. I have two tables ORDER, and LINE_ITEM. I would like to build a single screen with the ORDER table being the parent of 1 - M LINE_ITEM records.

I would like to edit an existing form rather than create this from scratch. Any ideas where I could find a form like this?
