PLEASE HELP! "FRM_!)_at_@!: cannot read file testmenu(file name)"

I run the 30 days trails PO8 and Developer2.1. I used From build and created a very simple emnu and form. I attached the menu to the form. It show me "FRM - 10221: cannot read fole testmenu" error. If I only run the form, it works fine.
Anyone knows where I am wrong, please help me out!!!
