Michael,
You can't convert from menu50 to forms45 in one go. Conversion path: menu50 > forms40 > forms45 You must create the forms40 repository first. All scripts needed to bild/grant forms40 and forms45 can be found in $ORACLE_HOME/forms45/admin/sql
