Basing a data block on a procedure, FORMS 5

Forms 5, allows a data block to be based on a pl/sql procedure. I cannot find any examples of this. Please can someone point me in the right direction, alternatively can I have a copy of a database procedure someone has written for them selves (probably just the declarative part).
