I have not come across an add-in that will work with forms in such a manner.

www.kofax.com is one of the best solutions for your needs, though there are others.

I have munged together a rudimentary scanning solution, w/scanning software that came with the scanner, and forms built-ins, however this solution is not vary practicle.

Another thing you should be aware of: Forms doesn't support multi-page tiffs, and has a limit of 64k when working with tiffs.

For these reasons, forms is not a viable for an imaging solution.

(An associate of mine told me that Oracle support suggested using JPG format for an imaging solution w/forms. Because of this, I spent many hours trying to convincing him how bad of an idea that is. The worst part is that I suspect he still doesn't believe me. :-| )
