HELP!!! Web Forms problems

Hello,

  Can someone with experience with Web deployment of forms help me out? My current setup is Developer Server v2.1, Oracle 8, and OWAS 3.0.1 on an NT 4.0 server machine, and I've developed on a Win95 desktop a Forms 5.0 form, which has been moved to the NT machine. I've attempted to configure everything properly (set up a listener, set up the virtual path/physical path mappings for Java classes, Java archives, and HTML, set up a forms cartridge, customized the base html page which gets downloaded, etc.), but I when I try to access the page (both directly and through the forms cartridge) I get the following Java error messages:

FRM-99999: Failed to connect to the server: {name_of_my_server}:9000 Details.....Java Exception:
oracle.forms.uiClient.v1_4.engine.RunformException: FRM-99999: Failed to connect to the server: {name_of_my_server}:9000

at oracle/forms/uiClient/v1_4/engine/Runform.initConnection (Runform.java)
at oracle/forms/uiClient/v1_4/engine/Runform.startRunform (Runform.java)
at oracle/forms/uiClient/v1_4/engine/Runform.run (Runform.java)

Any suggestions? Can anyone spot a setup step that I am missing? I am quite new to Web deployment, so there could well be something obvious that I am not even aware of. Thanks.
