You can't use PL/SQL tables within Forms v4.5 as it only supports PL/SQL v1
constructs and PL/SQL tables didn't come to life until PL/SQL v2 and above.
To work around this restriction, we have always used Record Groups where we
would normally want to use PL/SQL tables as they can be structured in the same
way so you can try this instead.
