I struggled with the same questions that you had for a few months. Apparently, nobody knows the answer on this list (you can see prev. posts from me re: the same subject, with no replies). But, I do have a suggestion:

The ONLY person in the WORLD who apparently knows the answer to this question is located at Oracle Support. You need to open a TAR, and then request a white paper (not availabile online) called 'Enterprise Methods and Tools - Practice Development. Developing and Generating Forms 5 Using Blocks Based on Stored Procedures'. With some luck, they'll fax it to you, then you'll have part of the answer.

I would repeat the answer here, but it would be very, very lengthy & I haven't got it to work yet myself.
