If you convert Forms 4.x to 5.0 there is a conversion of all PL/SQL from PL 1 to PL 2. I did a small test on an existing Forms 4.5 to 5 and it did change two of our PL/SQL code. Not big change. It was still working. If your forms are very complex... you work may be bigger.

For your RDBMS, if you have 7.0 you may have to upgrade it to 7.x (1, 2 or 3) before going to 8.0. Verify this.
