  1. may be You haven't generate the menu.mmx. To do that in Object navigator window, place the mouse cursor to the menu , and press ctl-T
  2. you didn't set the FORMS45_PATH, to do that call program regedit (NT4.0) or regedt32 (NT3.51) , IN HKEY_LOCAL_MACHINE\SOFTWARE\ORACLE\FORMS45_PATH add you working path.
