hmm - the form will not appear within the browser, it will open up a new window (at least that's the way it works for me..). The browser will have a little gray box in it.

Do you have Netscape 4.04 with JDK1.1.4 softupdate installed? You need the correct version of Java for it to work at all.

Can you make it work with the appletviewer?
