conversion problem from 4.0.13.15 to 5.0.5.4.0

Any help or comments appreciated.

We are upgrading from forms 4.0.13.15 to 5.0.5.4.0 (at least we tried) lots of forms contains database form reference. first installed 5.0 tables and saved referenced form to database under same user.
when referencing forms opened, designer says than can't find module. Ok ! accept situation and try to save form... then f50des32 generates an access violation.
Technical support can't offer a solution. any ideas ?
